Conference Fees (Friday, May 4)

Members

• $145 - Early Bird;

• $180 (after March 31, 2012)

Non-members

• $180 - non-members Early Bird

• $215 non-members (after March 31, 2012)

• Group rate - $160/person for groups of four or more people
  In order to obtain the group rate at the registration desk, you must present an invoice order signed by your employer. An individual registration, once purchased, cannot be transferred to a group registration.
  

Note: All above conference fees include $25 MADLaT membership

Student Rate

• $55 per full-time student (Student card required at door)

Note: All above fees are per person.

Refund/Cancellation Policy:

Written notice to MADLaT at madlat2012@madlat.ca is required. A $25 cancellation fee applies. No refunds after May 2, 2012

Step 1: Complete Conference 2012 Registration Form (Access below)

Step 2: Submit Payment

Option (a) – Credit Card Payment - Online only.

NOTE: To pay by credit card, you need a PayPal account.

If you do not have one, go to the credit-card payment page and click on the PayPal "Buy Now" button.

You will be guided through the process of obtaining one.

There is no cost associated with the creation of a PayPal account.

Option (b) – Invoice. Send by fax a copy of your completed registration form including invoicing information to 204-667-5584. Invoices must be paid by cheque only.

Option (c) – Cheque. Send a copy of your completed registration form and cheque to:
